  • Each provider on the team.
  • Every specific procedure they are authorized to perform.
  • The date their competency was verified for that procedure.
  • The signature of the delegating medical director.

This document is your proof that you have a safe, structured process for patient care and is one of the first things a medical board investigator would ask for in an audit.
